The following list of county school superintendents was updated March 2010.

Homeschooling families are required to file an Affidavit of Intent to Homeschool within 30 days of starting to homeschool for children ages 6 to 16.  Please mail your Affidavit of Intent to Homeschool to the County School Superintendent for the county in which you live.

Many counties have their own version of the Affidavit of Intent to Homeschool.   However, it should be noted that a specific form is not required by law.  We encourage families to use the affidavit provided by AFHE website as it includes only the information required by law. 

Proof of birth must be submitted with the notarized affidavit.  The most common proof of birth is the birth certificate.   Please note that a certified copy of the birth certificate (not a photocopy) must be submitted.   Please visit the AZ Law page for other acceptable proofs of birth if a certified birth certificate is not available.
